FORM 1 and LRIZ FORM 1 are the same screen. They describe the facility for the reporting year: how it operated, what it consumed, and what it produced. LRIZ is the local-register variant; NRIZ assigns which obligation appears.

Why this form is sent #

The Agency needs a yearly picture of the site before emission and waste figures can be interpreted: working regime, fuels, products, activity totals, and raw materials. Even a year without production still requires a report when the obligation is active.

When it applies #

The form can be created when the facility is active and NRIZ has assigned FORM 1 or LRIZ FORM 1. A facility does not have both at once. One form exists per company, facility, and year.

What to fill in #

1. General — company, facility, year, working regime, statement of responsibility.
2. Additional data — operational indicators required for that facility type.
3. Fuels — fuels used during the year.
4. Products — products manufactured.
5. Quantity by activity — totals split by activity.
6. Raw materials — main inputs.
7. Note — optional explanation.

Company, facility, and year cannot be changed after the first save.

Specifics #

Seasonal regimes need a start and end of the season. Empty tables are acceptable only when the facility truly had no corresponding activity — the rest of the profile must still be complete.
