FORM 4 and LRIZ FORM 4 are the same screen. They report pollutants in waste or discharges that reach soil from one soil pollution source at a facility.
Shared steps: How forms work.
| Who | Facilities with a soil-emission obligation on a source |
| Submitted for | Facility + soil pollution source |
| Frequency | Annual |
| Deadline | 31 March for the previous calendar year |
Why this form is sent
Releases to soil are recorded separately from air and water. The form documents which pollutants reached the ground, in what amounts, and in what waste or disposal context.
When it applies
The soil source must belong to the facility and be active, with FORM 4 or LRIZ FORM 4 assigned. One form exists per company, facility, source, and year.
What to fill in
- General β company, facility, year, soil source, statement of responsibility.
- Additional data β waste or disposal context for that source.
- Soil emissions β one or more pollutant rows (substance and quantity).
- Note β optional explanation.
Specifics
The same pollutant should not be listed twice on one form. At least one emission row is expected when the source operated. If the source did not discharge in the year, that should be clear from the additional data and the note rather than from duplicated empty rows.
