FORM 5, LRIZ FORM 5, and GIO1 share one screen. They report waste generated at the facility during the year: catalogue code, quantities, and what happened to that waste.

Why this form is sent

National waste statistics and inspections start from what the site produced. Each waste type is a separate form so quantities and management routes stay traceable.

When it applies

NRIZ assigns FORM 5, LRIZ FORM 5, or GIO1 β€” not a combination of FORM 5 and GIO1 on the same facility. FORM 1–5 cannot be combined with LRIZ FORM 1–5.

A new form is created per waste catalogue index for that facility and year. The same index cannot be added twice.

Where FORM 5 or LRIZ FORM 5 is active, a DKO obligation is typically also expected for hazardous-waste movements. See Daily waste (DEO6 / GIO6, DKO).

What to fill in

  1. General β€” company, facility, year, waste catalogue index, statement of responsibility.
  2. Classification β€” origin, type, character; H and Y codes when the waste is hazardous.
  3. Components β€” composition of hazardous waste, when required.
  4. Quantity β€” generated, stored, and related amounts for the year.
  5. Management β€” disposal, recovery, export, or other routes used.
  6. Note β€” optional explanation.

Classification is often copied from the previous year for the same index and can be adjusted.

Specifics

Quantities and management routes must match: waste that was generated has to be accounted for (stored, handed over, treated, exported). Hazardous codes must match a hazardous catalogue index.

Handling at specialised plants (disposal, reuse, import, export) is reported on waste handling (GIO2–GIO5) forms, not on this production form.
