Why this form is sent
Municipal waste is tracked separately from industrial production waste. The Agency needs coverage of settlements, seasonal quantities, composition analyses, and the final destination of the collected waste.
When it applies
The facility must be active with KOM1 assigned. The same facility can have several KOM1 forms in one year, one per municipality. There is only one form per company, facility, year, and municipality.
What to fill in
- General β company, facility, year, municipality, statement of responsibility.
- Equipment and vehicles β bins, vehicles, and related collection assets.
- Settlements β places served; from 2024 some settlement totals are split and calculated by the portal.
- Quantities β municipal waste amounts, including the mandatory municipal-waste category row.
- Composition β analysis results when they exist for the year.
- Management / hand-over β where the waste went.
- Note β optional explanation.
Specifics
Mean values in quantity and composition blocks are calculated by the system; they are not typed in. For 2023 and earlier, some settlement totals are entered directly instead of the later split fields.
Industrial waste generated at the site is FORM 5 / GIO1, not KOM1.
