Why this form is sent
The Agency tracks what leaves the site with wastewater: measured concentrations, discharged volumes, and β where relevant β how water is supplied and how sewage is handled.
When it applies
The water source must belong to the facility and be active, with FORM 3, LRIZ FORM 3, or OTV assigned. One form exists per company, facility, outlet, and year.
OTV is not used for reporting years 2022 and earlier. For those years the FORM 3 / LRIZ variant applies when assigned.
What to fill in
- General β company, facility, year, water source, statement of responsibility.
- Additional data β operating context of the outlet.
- Wastewater measurements and annual balance β measurement events, values, and yearly discharged volume when a yearly balance is created.
- Recipient measurements β measurements in the receiving water, when required.
- Water supply / sewage β only when those sections apply to the outlet.
- Note β optional explanation.
Specifics
Creating a yearly balance unlocks extra fields (discharge regime, period, total volume, how the volume was determined). Those fields stay hidden until the balance option is used.
